A Hospital without Borders, Standardising the Medical Referral System with Digital Tools

"Ding Dong!" Landseed International Hospital (hereinafter Landseed) receives a Line@ message: "Another hospital has a patient who needs a cerebral artery thrombectomy, can you take the patient?" The doctor on duty presses "Confirm Admission," and the ambulance is dispatched immediately. Minutes later, a notification pops up: "Ambulance will arrive shortly," signalling that the Landseed thrombectomy team is ready for arrival of the patient.

Well-Coordinated Medical Referral System
Landseed is one of the main referral hospitals for cerebral artery thrombectomy in the Taoyuan, Hsinchu and Miaoli areas. In the past, communication between staff could be cumbersome when receiving transfer requests from other hospitals. To ensure the security of patient information and to avoid delays in communication, Landseed standardised each step of the thrombectomy referral process and integrated it into the familiar communication context of LINE@ for physicians, creating a referral system for which Landseed is primarily responsible.

By implementing a 24-hour central monitoring process, Landseed connects IA Thrombectomy and information technology, ensuring timely transfers and actively improving patient prognosis. The referral system also incorporates an AI system for real-time interpretation of brain images.
目Currently, this system has been implemented at Landseed and hospitals in Taoyuan, Hsinchu and Miaoli that have joined Landseed's ischaemic stroke communication network. It will be gradually introduced to partner hospitals. Based on the analysis of the actual medical schedule of the transferred patients, it significantly speeds up direct discussions between the referring hospital and the in-house 24-hour on-call team. Even during holidays or at night, it integrates with the hospital's existing on-duty contact system, allowing the medical team to prepare for the patient's arrival and initiate rescue efforts promptly.
Community Care with Health Data Converged and Centralised
Landseed Hospital innovatively breaks down many medical and transfer processes, introducing LINE's digital tools so that doctors can quickly adapt without the need for additional app downloads. In the future, this approach can be extended to saving heart attack patients or other emergencies, and even provide guidance for citizen care, making community healthcare more accessible through mobile phones.
Currently, Landseed collaborates with the Department of Public Health, Taoyuan, serving approximately 13,000 members and connecting with 68 remote measurement stations and 13 health centres. These locations offer free blood pressure measurement, manage hypertension cases, screen for atrial fibrillation (AF), and can upload information through measurement devices or LINE. When patients visit, doctors can access comprehensive data from both inside and outside the hospital to better inform their diagnosis.
Exercise Prescription for Corporate Members and Their Employees
Landseed has also brought the exercise prescriptions that companies need most to mobile devices. In collaboration with the Landseed Sports Medicine Center team, they developed a platform for exercise prescriptions to assist companies in caring for the health of their employees. Exercise can improve chronic pain, and the platform's standout feature is that all exercise prescriptions can be confirmed for proper execution through precise imaging, ensuring the desired prescription effects. Additionally, it allows detailed tracking for professional exercise management to achieve the goal of alleviating chronic pain. This service will be showcased at the Healthcare+ Expo.

◎Multilingual Simulation Training for Medical Professionals
Landseed, in collaboration with Medomic Healthcare Technology, has established the Virtual Clinical Practice Training and Examination Center. Technology creates virtual patients, virtual hospitals, and virtual examiners, breaking the limitations of time and space. This enables medical professionals to practice safely and conveniently, addressing patient issues and enhancing interdisciplinary clinical practical skills.

This center also provides international users with multilingual exercises, presenting realistic scenarios of diverse patient conditions through digital platforms. Practitioners can perform any desired tests and examinations, achieving a new realm of remote and flexible participatory learning.
The center can also replace the costly physical OSCE test facility. With a large number of candidates, it eliminates the millions of dollars in costs, manpower and time involved in administering the exam, while providing a high-quality examination. An annual national competition for clinical skills, organised by major medical schools and hospitals, will be hosted by the College of Medicine of National Cheng Kung University for the 6th edition in October 2023, with nearly 200 outstanding medical students and specialist nurses expected to showcase their skills.
◎Inpatient Critical Illness Risk Prediction System
While many hospitals have Clinical Alert Systems (CAS) and Early Warning Systems (EWS), there is no international standard on how to proactively identify the risk of critical illness in hospitalised patients. Landseed is working with the National Yang Ming Chiao Tung University, integrating various data aspects to develop an AI model prediction system. The system is currently deployed in the hospital and is continuously improving its data quality.
This system can predict the likelihood of a critical risk occurring in hospitalised patients within the next 6 hours, enabling healthcare professionals to intervene proactively. Since its launch almost six months ago, it has successfully predicted critical risk in 136 people, allowing the hospital to respond promptly.

◎AI-aided Snoring Expert
Snoring loudly during sleep doesn't necessarily mean you're sleeping well. Snoring is a common sleep problem that not only disturbs the sleep of bed partners, but can also pose chronic health risks, such as being a sign of sleep apnoea. In contrast to current sleep tests, which require numerous adhesive patches on the head and body to monitor sleep conditions and involve a night in hospital, Landseed has collected data on hundreds of snoring sounds as material and conducted research by comparing these with polysomnography (PSG) data from several physiological sleep tests. Landseed has further developed and trained an AI snoring analysis model, ultimately creating an AI-aided snoring analysis system.
People can provide snoring data recorded at home, coupled with Landseed's AI non-contact sleep analysis radar device, developed in partnership with Wistron. This allows healthcare professionals to analyse snoring and sleep apnoea during sleep at home, providing detailed reports and recommendations to improve treatment. The system integrates with sleep and chronic disease management, providing sleep tracking and care services. In the future, the system is expected to be used in sleep centres in various hospitals and for sleep screening in large companies.
